• Parent/Guardian Consent Form COVID-19 Pandemic – In-School Mentoring (ISM) Program Contact Temporary Policy Change
  • Standard ISM program policies dictate that there be no contact of any form between Mentors and Mentees outside of the weekly school visits.
  • Due to the COVID-19 pandemic, we are offering our matches a temporary exemption to this policy.
  • By signing below, you are indicating your consent for your child to engage in phone and virtual contact with their In-School Mentor until which time the match can resume in person visits at the school (during the school day), up to and including the last day of school, June 27th or June 29th, 2022 (depending on School Board). Virtual contact will be through a School Board approved platform.”
  • I understand that:
    • In addition to my consent as indicated here, my child’s In-School Mentor must also provide written consent to the agency. I understand that both consents must be received before my child can engage in phone or virtual contact with their Mentor
    • As the parent/guardian, I will have the final say on what forms of communication my child will or will not engage in with their mentor. For example, I may agree to phone calls and texting, but not social media contacts. I will communicate any limitations with both my child’s Mentor and the agency
    • This is a temporary exemption to the standard contact policies (see above)
  • I agree to:
    • Provide supervision and support during contact, and to check in with my child following phone and/or virtual contacts with their mentor to see how things went
    • Contact BBBS immediately should any concerns arise related to my child’s match
    • Return to the normal contact policies (see above) once schools re-open and in-person visits are expected to resume
    • Contact the agency/my Program Coordinator if we need support in adjusting to this new form of mentoring
    • Do my best to support weekly contact between my child and his/her mentor, but will ensure that contacts do not occur more than once per week (initiated by either party)
    • Participate in all regularly required match monitoring check-in’s as well as any additional match monitoring check-in’s requested by the agency
