• In-School Mentoring (ISM) Program Contact Temporary Policy Change
  • Standard ISM program policies dictate that there be no contact of any form between Mentors and Mentees outside of the weekly school visits and approved agency events.
  • Due to the COVID-19 pandemic, we are offering our matches a temporary exemption to this policy.
  • By signing below, you are indicating your consent to engage in phone and virtual contact with your ISM mentee until which time the match can resume in person visits at the school (during the school day), up to and including the last day of school, June 27th or June 29th, 2022 (depending on School Board). Virtual contact will be through a School Board approved platform.
  • I understand that:
    • My mentee’s parent/guardian must first provide the agency with written consent for our match to engage in phone or virtual contact before we engage in any such contact during the school closures
    • The parent/guardian will have the final say on what forms of communication we will or will not engage in
    • This is a temporary exemption to the standard contact policies (see above)
  • I agree to:
    • Return to the normal contact policies (see above) once schools re-open and I have been advised to resume in-person visits
    • Contact my Program Coordinator if I need support in adjusting to this new form of mentoring
    • Do my best to contact my mentee by phone or virtually once per week
    • Participate in all regularly required match monitoring check-in’s as well as any additional match monitoring check-in’s requested by the agency
